What this service stores

The service uses a signed anonymous ownership cookie so your browser can access its own forms. It stores uploaded blank templates, field layouts, saved form names, payment evidence, and operational logs.

The service also keeps low-cardinality daily totals for form creation, editing, analysis, checkout, payment, ZIP preparation, and failures. These totals contain no raw event history, visitor identifier, URL, referrer, or user-agent and are retained for up to 24 months. The functional ownership cookie is not used for analytics.

Do not upload patient information or completed clinical records. This service is designed for blank form templates only.

When an eForm ZIP is successfully prepared for download, the service automatically keeps a separate example for improving and training its form-detection algorithms. This includes the blank template images, available starting field layout, final exported layout and ZIP, and differences between the layouts. Every distinct exported version may be retained. Collection is automatic and has no opt-out.

Processors and access

Document images may be processed by third-party software companies. Uploaded pages are owner-gated and are not public static files.

Automatic drafts expire after seven inactive days. The first manual save sets a fixed 30-day unpaid deadline. Paid purchase evidence and forms remain available unless access is revoked or the operator must remove them by law.

The algorithm-improvement examples are private, are retained indefinitely, and are separate from your saved forms. Deleting or removing a saved form does not delete its previously retained examples. These examples are not publicly accessible. Contact support about an example that must be removed.

Your responsibilities

Use only blank templates that you are allowed to process. Review every detected field and every generated ZIP before clinical use.

Generated output is a drafting aid. It is not medical advice and is designed only to work with the current version of OSCAR Pro.

Service limits

The service may reject unsafe, oversized, expired, abusive, or unsupported requests. Provider outages can delay analysis without changing your payment rights.

Case-by-case review

Refund requests are reviewed case by case and must be requested within 14 days of purchase. Duplicate charges and unresolved material delivery failures are normally eligible.

A change of mind after a successful download is not eligible. Approved refunds revoke download and recovery access. Any rights required by law still apply.

How to get help

Contact support with the form name, the approximate purchase time, and the Stripe session identifier shown in an error. Do not send patient information, completed forms, recovery keys, or payment card details.

For a delivery problem, keep the recovery key private. The owner-only recovery action can show the same key again for eligible paid purchases.
